RESPONSIBILITIES

Career Counseling and Evaluation

  • Administer and score vocational tests
  • Perform Career exploration and testing services according to guidelines provide in OOD VR Provider Manual.
  • Interpret and explain testing and exploration results to participant and VRC and provide service recommendation related to result of counseling or evaluation activities.
  • Coordinate testing and exploration activities that will allow participants to gain an understanding of identified potential vocational goal options.

Job Seeking Skills Training

  • Teach Job Seeking Skills Training (JSST) such as application completion, interviewing skills, networking skills, resume and cover letter development, how to look for employment and where to look for employment, how to follow-up with employers, address question regarding disability, needed accommodations and any other skill that are necessary to obtain employment.
  • Meet with the participant to complete required list of job prep functions as indicated in OOD VR Provider Manual guidelines.
  • The resume, sample application and job search materials should be exceptional and meet Matrix and OOD standard.
  • Perform follow-up with Vocational Rehabilitation Counselor and with participant to ensure service requirements are met and participant is satisfied with service provision.

Job Search Program

  • Attend all weekly job search appointments with participant.
  • Provide at least 4-6 job leads per week using internet or whatever means necessary to provide appropriate and viable job leads. All job leads should fall within the participants abilities and functional levels.
  • Provide Job development services per Ohio OOD VR Provide Manual guidelines.
  • Provide follow up via e-mail or phone call to VRC to ensure that counselor is update and well informed about participants progress.
  • Assure that the participant is compliant with the program and report to the VRC any significant issues.
  • Comply with the standards set forth by Matrix Vocational Solutions.
  • Perform other related duties as assigned.

Case Documentation

  • Score test in accordance with testing manual guidelines.
  • Complete legible well written reports on each case in accordance with requirements specified by OOD in VR Provider Manual.
  • Enter billings and outcomes information into Matrix systems as specified by Matrix Policy and Guidelines.
  • Follow all documentation guidelines in compliance with CARF.

QUALIFICATIONS

  1. Bachelor’s Degree in the Human Services or Health Care Field.
  2. Certification or Licensure such as CRC, PC, LSW, LISW.
  3. Thorough knowledge of the Rehabilitation field, trends and methods.
  4. Must have thorough knowledge of disability.
  5. Must have a valid driver’s license with reliable transportation and willing to travel.
  6. Must have excellent communication skill and problems solving abilities.

Nurse Case Manager - Workers' Compensation - Cincinnati, Cleveland, Columbus in OH and Northern Kentucky

BASIC FUNCTION

Matrix Vocational Solutions is actively recruiting for a Nurse Case Manager position to assist our client’s injured workers with case management and return to work services.

RESPONSIBILITIES:

  • Perform both telephonic and field case management services
  • Assist with Triage of initial claims and provide case direction to Matrix AE and employer
  • Perform case assessment and develop action plans to support recovery and return to work
  • Manage cases toward successful return to work outcomes
  • Coordinate needed services and maintain timely communications
  • Cultivate excellent relationships with all parties (Matrix AE’s, IW’s, and Matrix clients)
  • Provide written reports on case status and updates (post physician visit/weekly/monthly)

JOB QUALIFICATIONS:

  • Prefer experience in both Telephonic and Field Case Management – preferably in workers’ compensation
  • Active Registered nurse (R.N. License and possess the ability to be licensed as a registered nurse in multiple states without restrictions).
  • Applicants should be seasoned professional nurse with at least 3 years of clinical nursing experience and at least 1 year case management experience
  • Understanding of case management processes
  • Excellent interpersonal communication skills – both oral and written.
  • Computer skills & experience with Microsoft word and Excel.
  • Full time and part time work schedule available
  • Must be credentialed with one of the following: CDMS, CRRN, COHN, or CCM

Nurse Triage - Telephonic Case Manager (Part-Time) - Ohio

RESPONSIBILITIES:

  • Triage new workers’ compensation injury claims to determine severity and facts surrounding injury
  • Perform 4-point contact: Injured Worker, Treating Providers, Employer and Matrix Account
    Executive
  • Telephonically interview injured worker to assess injury and medical treatment (assist Injured Workers
    with the workers’ comp system/process)
  • Request, secure and review all medical treatment reports (Urgent Care/ER visits, Office Visits,
    Diagnostics, etc.)
  • Identify medical issues and provide written update to Matrix Account Executive and Employer
    Using the Official Disability Guidelines (ODG), provide estimated healing/treatment duration
  • Using the Official Disability Guidelines (ODG), provide estimated healing/treatment duration
  • Communicate “red flag” issues to Matrix Account Executive & Employer
  • Recommend additional case management interventions (ongoing Telephonic or Field Case Management)
  • Telephonic Case Management: Monitor treatment, authorize appropriate treatment, manage cases to return to work (light duty), secure maximum medical improvement, etc.

JOB REQUIREMENTS:

  • Experience in medical Nurse Triage – workers’ compensation telephonic case management preferred
  • Active Registered Nurse or Licensed Practicing Nurse
  • Understanding and working knowledge of Official Disability Guidelines (ODG)
  • Seasoned professional nurse with at least 3 years clinical nursing – workers’ compensation case management preferred

Return to Work Assistant - Cincinnati, OH

BASIC FUNCTION

Provide assistance to our Return to Work Coordinators in finding temporary job placement for injured workers at nonprofit facilities.

RESPONSIBILITIES:

  • Assigning and entering referrals
  • Entering restrictions and creating call lists
  • Research and identify nonprofit facilities to place injured workers
    for volunteer job duties
  • Ensure case manager coverage for initial nonprofit appointments
  • Perform reminder calls to injured workers for upcoming meetings
  • Weekly contact with nonprofit supervisors for open cases
  • Bi-weekly contact with injured workers for open cases
  • Documenting all injured worker and nonprofit correspondence
  • Electronic timesheet system maintenance
  • Assisting in troubleshooting issues
  • Mailing on an as needed basis
  • Other duties as assigned
